NEW PATIENTS

Cahaba Medical Care is always accepting new patients at each of our clinic locations.

We accept all insurances, including Medicaid. We also accept uninsured patients. Our comprehensive services are available to everyone - regardless of your ability to pay. **PLEASE NOTE: Cahaba Medical Care does not prescribe and/or authorize refills on any medications deemed a controlled substance during a patient’s initial visit, regardless of whether or not the patient has a prior prescription from another physician.

WALK-INS

Cahaba Medical Care does accept walk-in patients for sick visits. We try our best to accommodate everyone and can offer you a visit within that same calendar day if needed. However, please be aware that walk-in patients will be asked to wait until the next available appointment as patients with scheduled appointments will be seen first. Also, as a walk-in patient, you may not be able to see your primary doctor and may instead be seen by one  of his or her partners, depending on which physicians are in the office and seeing patients on that particular day.

 

SCHOOL/SPORTS PHYSICALS

CMC does perform yearly school / sports physicals. We use these appointments to not only clear a patient for school attendance / sports participation, but to also verify that he or she is up to date on all immunizations and other health maintenance measures.

If you would like to be seen at CMC for a sports / school physical, we ask that you first call to schedule an appointment. You do not have to be an established patient at our clinic to be seen for a physical. However, if you are not a current patient at CMC, please have your insurance card available when you call for an appointment so that our billing office has time to verify your insurance and ensure your office visit will be covered by your insurance. If you do not have insurance, you can still be seen for a school / sports physical, but you will be asked to pay a fee at the time of service.

At the time of your physical, please bring your updated immunization form (“blue card”) with you to the appointment.

IMMUNIZATION FORMS

You may call or come in to the office to request an immunization form (“blue card”) if needed. Once you have submitted your request, the blue card will be ready for you to pick up within 72 hours.

 

FORMS/PAPERWORK

CMC is more than happy to assist you in completing any necessary paperwork that affects your medical care. However, we ask that you please allow us a minimum of three working days to get the paperwork completed, signed and ready for pick up. Some paperwork, including motorized wheelchair / scooter evaluations, disability forms, and other forms that require an office visit, office notes, and additional time from the physician and staff, may require more than three days. PAST MEDICAL RECORDS/RECORD RELEASES

If you are transferring your care from another facility to Cahaba Medical Care, we ask that you please first come by one of our office locations to sign a Medical Records Release Form. It is important that your CMC provider is aware of your past medical history, including information regarding any previous or existing illnesses or conditions, as well as previous and/or current treatments and medications. Therefore, we will need to obtain all of your previous medical records for our providers to review before your initial appointment in order to provide you with the safest and highest quality of care. Our office will fax your signed release to your previous physician’s office, and once we have obtained you records, we will then call you to set up your initial appointment. Please be aware that this process can take anywhere from a few days to a few weeks.

If you are an established CMC patient and have recently seen another physician, been seen in the ER, or been admitted to the hospital, we ask that you please sign a Medical Records Release Form for each facility you visited. Our providers will need to review and maintain these records in order to best care for you moving forward.

TORNADO WARNINGS/SCHOOL CLOSURES

In the event of most bad weather conditions, Cahaba Medical Care will remain open and operate under normal office hours. However, in the case of severe weather or a tornado warning, the office will close for the safety of the our patients and staff.
