CAHABA MEDICAL CARE IS RECOGNIZED AS A LEVEL 3 PATIENT-CENTERED MEDICAL HOME (PCMH) BY THE NATIONAL COMMITTEE FOR QUALITY ASSURANCE (NCQA).


WHAT IS A PATIENT-CENTERED MEDICAL HOME (PCMH)?

The Patient-Centered Medical Home (PCMH) is a care delivery model that puts patients at the center of care with a focus on building partnerships between patients and their primary care providers. CMC uses evidence-based guidelines to care for our patients in a team setting. As your medical home, we are able to coordinate treatment and services with an interdisciplinary team of medical professionals, including physicians, physician assistants, nurse practitioners, nurses, social workers, radiology technicians, dietitians and more. Most importantly, YOU (the patient) will play a critical role on the care team. The PCMH approach helps our patients navigate the healthcare system while allowing us to provide a broader scope of clinical expertise.

THE PCHM ENCOMPASSES FIVE KEY PRINCIPLES:

  • Patient-Centered - building partnerships between the patient and provider where each patient’s culture, values, unique needs and preferences are respected; ensuring that each patient is fully informed and empowered to assist in establishing treatment plans and manage their own care.

  • Comprehensive - providing the necessary care to meet the majority of each patient’s physical, spiritual, social and emotional healthcare needs, including prevention and wellness, as well as acute and chronic care.

  • Coordinated - Organizing and communicating care across all spectrums and settings, including subspecialty care, inpatient and outpatient hospital care, home healthcare, community services and more.

  • Accessible - providing services that are easily accessible with shorter wait times, extended clinic hours, multiple methods of communication, and around-the-clock electronic and/or telephone access to a provider.

  • Quality and Safety - demonstrating an ongoing commitment to safety and quality improvement through continuous performance measurement; evaluating and responding to patient experiences and satisfaction; patient education and support tools to encourage shared clinical decision making; promoting health management across the general population; sharing quality and safety data; and more.

According the the National Committee for Quality Assurance (NCQA), “a growing body of scientific evidence shows that PCMHs are saving money by reducing hospital and emergency department visits, mitigating health disparities and improving patient outcomes.”
